Hazard Mitigation Plan Public Survey
There are two ways you can fill out our public survey. Below is a link to the online version of the survey. There is also a hard copy of the survey that can be printed and filled out by hand. Hard copies can be returned to either the physical address or the email address listed on the form. Both versions of the survey contain the exact same information, so there is no need to fill out both!
